Topic: From Volume to Value: How Health Systems, Practitioner and Post-Acute Care Providers Navigate Value Based Care

  •  Details: Many healthcare organizations are partnering with practitioners and post-acute care providers to navigate the shift from volume to value. In this program, participants will learn how health systems are coordinating care with those providers through care pathways and technology. Participants will hear from thought-leaders who have led movements in their organizations to improve transitions of care. Specific focus will be given to the Center for Medicare and Medicaid Service’s (CMS) Bundled Payment for Care Improvement (BPCI) program and the JenCare model. This program will also take a closer a look at how health systems partner with organizations like Remedy Partners to improve patient outcomes.

  • Details:   Staff retention continues to be a top priority for most health care executives. Health care organizations rely on the support of engaged and loyal clinical workforce to support their mission. Over the last several years health care organizations across the country have faced the significant impact of nurse shortages. High nurse retirement rates, changing demographics, insufficient staffing and the impact on stress levels of nursing are a few of the many trends impacting nurse shortages. Retention efforts are critical to reduce the burden of nurse shortages on health care organizations.  This program will take a closer look at how local health systems are recruiting and retaining staff to position their organizations for continued success.
