Quality Management System Model for Respiratory Services (HS4-A2); Quality Management System Model for Medical Imaging Services (HS5-A2); Application of a Quality Management System Model for Inpatient Medication Use (HS10-A2)

CLSI submits the following for vote as candidate-for-advancement consensus documents.  The documents and ballots are available to delegates and alternates of Active and Associate Active member organizations through CLSI Forums.  

Second Notice

HS4-A2Application of a Quality Management System Model for Respiratory Services; Approved Guideline—Second Edition
This guideline describes the respiratory services path of workflow and provides information for respiratory services operations that will assist the services in improving their processes and meeting governmental and accreditation requirements.  HS4-A2 provides practical examples of how to apply the path of workflow model and the QSEs to the pulmonary diagnostic laboratory and respiratory therapeutic services. When used with CLSI/NCCLS document HS1—A Quality Management System Model for Health Care, the guideline can provide the means for respiratory services to implement a complete quality management system.  The deadline for the completed ballot for HS4-A2 to be received at the CLSI offices is 30 January 2006.

HS5-A2Application of a Quality Management System Model for Medical Imaging Services; Approved Guideline—Second Edition
HS5-A2 describes the imaging services path of workflow and provides information for imaging services operations that will assist in improving the services’ processes and meeting governmental and accreditation requirements. This document, when used with CLSI/NCCLS document HS1—A Quality Management System Model for Health Care, can provide the means for a medical imaging service to implement a complete quality management system. Individual service areas, such as diagnostic radiology, computed tomography (CT), ultrasound, interventional radiology, magnetic resonance imaging (MRI), mammography, and nuclear medicine can apply this model to their respective operations.  The deadline for the completed ballot for HS5-A2 to be received at the CLSI offices is 30 January 2006.

HS10-A2Application of a Quality Management System Model for Inpatient Medication Use; Approved Guideline—Second Edition
This guideline describes the hospital inpatient medication system path of workflow, and provides information for medication system operations that will assist the hospital pharmacy in improving its medication-related processes and meeting governmental and accreditation requirements. HS10-A2, when used with CLSI/NCCLS document HS1—A Quality Management System Model for Health Care, can provide the means for a pharmacy to implement a complete quality management system.  The deadline for the completed ballot for HS10-A2 to be received at the CLSI offices is 30 January 2006.


Under our Administrative Procedures, a CLSI document approved by the area committee at the first level of the consensus process is submitted to delegates as a “candidate-for-advancement” consensus document. This begins the voting and approval period by the delegates.

As a delegate, your vote is to affirm (or reject) the document for advancement as a CLSI document. Please send your completed ballot to the CLSI offices by the above deadline.

In the consensus process, comments are invited at each publication stage. In each edition, the responsible committee includes a summary of comments on the prior edition and its responses to them. Any comments received on a candidate-for-advancement consensus document as a result of delegate voting and consensus review will be addressed by the committee during the document’s advancement.
