Critical incident reporting in managed care

Having a streamlined and consistent process for providers' critical incident reporting will be important under Iowa's managed care program, slated to launch April 1, 2016. Through IHCA's efforts with DHS, DIA, and the managed care organizations (MCOs), a single form has been developed for providers to use when reporting a critical incident.

The MCOs—Amerigroup Iowa, Inc., AmeriHealth Caritas Iowa, Inc., and UnitedHealthcare Plan of the River Valley, Inc.—developed a universal critical incident form, IA Health Link Incident Report Form, for all HCBS providers and case managers to use when submitting a major incident. This should help streamline the incident reporting process.

When a major incident has been witnessed or discovered, the HCBS provider/case manager must complete the form and submit it to the HCBS member's MCO in a clear, legible manner, providing as much information as possible regarding the incident. Contact and submission information for all of the MCOs is provided on the back of the form.

Critical incident reporting in Iowa Administrative Code (IAC) Chapters 77 and 90, remains a requirement for those eligible to participate in Home- and Community-Based Services (HCBS). Nursing facility and ICF-ID providers will be able to continue submitting incident reports to DIA as they currently do. The DIA system communicates with DHS and the MCOs, which means nursing facilities and ICF-ID providers do not need to do the additional submission on the IA Health Link Incident Report Form.
