NAHC submits comments on the OASIS-D burden

On March 12, 2018, the Centers for Medicare & Medicaid Services (CMS) announced in the Federal Register a request for comment in accord with the Paperwork Reduction Act for the Outcome and Assessment Information Set (OASIS)-D.  The OASIS-D was being modified to include changes pursuant to the Improving Medicare Post-Acute Care Transformation Act of 2014 (IMPACT Act). The changes to the OASIS-D were announced in the 2018 Home Health Prospective Payment System Rate Update Rule.

The OASIS-D item set is effective as of January 1, 2019, and includes the following changes:

  • New standardized items (GG0130, GG0170a-b, d-s, J1800, J1900) to support measurement domains mandated by the Act -Application of Percent of Residents Experiencing One or More Falls with Major Injury (NQF # 0674); and Application of Percent of Long-Term Care Hospital Patients with an Admission and Discharge Functional Assessment and a Care Plan That Addresses Function (NQF #2631).

  • Modification to OASIS items M0300/M1311 to support a new standardized pressure ulcer measure to replace the current standardized pressure ulcer measure. The new measure is Changes in Skin Integrity Post-Acute Care: Pressure Ulcer/Injury.

  • New items added for standardization to align with the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI), the Long-term Care Hospital assessment set, and the Minimum Data Set (MDS).

    • GG0100 M1900 Prior Function

    • GG0110 Prior Device Use

CMS also removed several data elements from the OASIS assessment at different time points.

In accord with the Paperwork Reduction Act, the comments submitted by the National Association for Home Care& Hospice (NAHC) focus on CMS’ burden estimate to collect and submit the data set.

CMS believes that the OASIS-D will be less burdensome for agencies to complete since many of the OASIS items have ben removed. However, the new items in the assessment tool could actually prove to be just as, if not more, burdensome to complete than the current OASIS C2.

NAHC challenges CMS’ burden estimate on three fronts: The time it will take for a clinician to complete the assessment tool; the burden associated with training cost; and the cost associated with the submission of the OASIS assessment.
