Family Nurse Practitioner - Sacopee Valley Health Center

Sacopee Valley Health Center has a position available for a full-time Family Nurse Practitioner to join our provider team.  SVHC is a multi-service, progressive, rural federally qualified community health center located between Portland, Maine and the White Mountains of New Hampshire. 

Sacopee Valley Health Center has a position available for a full-time Family Nurse Practitioner to join our provider team.  SVHC is a multi-service, progressive, rural federally qualified community health center located between Portland, Maine and the White Mountains of New Hampshire.  Services on site include integrated primary care, dental services, family planning, mental health counseling, psychiatry, nutritionist, optometry, podiatry, social services support, sliding fee coverage, care management, radiology and lab services.  Next Gen EMR.  Practice is outpatient with no OB.  On-call rotation is 4-5 times per month.  NCQA Level III PCMH.  This is a 4 days per week position with a competitive salary and benefits package including health, dental, disability and life insurance, 401k retirement plan, and a generous paid time off plan; Family Nurse Practitioners are eligible to apply for NHSC loan repayment.  Applicants must be a graduate of a Family Nurse Practitioner Program with current Maine licensure; at least one year of primary care medical experience preferred.  Please submit resume to nbuck@svhc.org or mail to: Human Resources Coordinator, Sacopee Valley Health Center, 70 Main Street, Porter, ME 04068.  Sacopee Valley Health Center is an equal opportunity provider and employer.
